Health Maintenance Program Service Standard 650-25-50

Health maintenance is a combination of services provided determine and maintain the health and well-being of older individuals.
Health maintenance services are available to individuals age 60 and older.
Health maintenance services are offered in a variety of settings including but not limited to senior/community centers, churches, adult care facilities, schools and cafés that meet the following standards:
- Federal, state, and local fire safety and sanitation codes/standards.
- Accessible for individuals with disabilities.
- Makes provision for a private area.
- Makes provision for a reception area/waiting area with adequate furniture to comfortably seat waiting individuals.
Health maintenance services are provided to individuals in their home if homebound.
Priority for health maintenance services must be given to older individuals:
- Residing in rural areas.
- With the greatest economic need (with particular attention to low-income older individuals including minorities with limited English proficiency).
- With the greatest social need (with particular attention to low-income older individuals including minorities with limited English proficiency).
- With severe disabilities.
- With limited English proficiency.
- With Alzheimer’s disease and related disorders with neurological and organic brain dysfunction and the caretakers of such individuals.
- At risk for institutional placement.
